Player Contract

To be completed for any child participating in playing football in the SYFL. Can be turned in to your local Chapter of your choice

Physical Form

MUST  be completed for every child participating in the SYFL ((Football and Cheer))

Cheerleader Contract

To be completed for any child participating in the cheer program in the SYFL. Can be turned in to your local Chapter of your choice.

Parent and Player Code of Conduct form

MUST be completed and signed by the Parent/Guardian for every child participating in the SYFL Football and Cheer.
